There are specific documents required at the time of application. Applicants must provide social security cards. Cards must be paper cards issued by the social security office for everyone in the household and no plastic cards will be accepted. A valid I.D. is needed for everyone 18 years and older. Expired I.D.s, licenses, or handicapped I.D.s are not accepted.

Applicants must have proof of income for everyone in the house receiving an income. Documentation examples include the last four check stubs, 2022 retirement benefit letter, VA social security and/or SSI award letter, and unemployment printout detailing the last 30 days of payments. Applicants must also provide utility bills. Bills can be electric, gas, or water bills from within the last 30 days. Bills cannot be disconnected or delinquent and the current bill cannot have a credit. The bill must have a name and service location, not a P.O. Box.

Proof of residency is required which can be a current bill in your name other than utilities if the I.D. or license does not have a current address. If there is no household income, the department will provide paperwork that must be filled out by everyone 18 years and older in the house. The paperwork must be returned within ten days.

If applying for someone else, paperwork must include a signed dated letter from the applicant that includes their phone number. Date of birth, race, sex, highest education level, and relation to the head of household must also be provided.
